Subject: Formal Letter with "accuse de reception"

Message: Hello Perhaps someone can answer this question for me. I have recently decided to return to the U.S. a bit earlier than planned, and so I need to get out of my lease. My lease states that I need to do this three months in advance, and that I need to send a formal letter 'avec recommendation', etc. Does anyone have a formula for such a letter. I would hate to write something only to have it be incorrect according to those picky French letter writing standards. Also, do I have to give a reason why I leaving the country or anything? Thanks!
